Located in leafy St John’s Wood on the Jubilee line, minutes from Central London, our luxury private Hospital has an international reputation for providing the highest quality of care to patients and is unique in that all profits fund our onsite Hospice, St John’s.

We are expanding our Hospice at Home service into Hammersmith Fulham and are seeking to recruit additional an additional Clinical Support Nurse.

What would be my responsibilities as a Hospice at Home Clinical Support Nurse

The Hospice@Home Service is designed to deliver end of life and respite care for patients with specialist palliative care needs that wish to remain in their own home. Care is delivered in the patient’s home by Health Care Assistants.

The post holder will be responsible for the co-ordination of the St John’s Hospice @Home service which provides home care for patients living with in our catchment area of Westminster, Kensington, Chelsea, South Brent and Hammersmith and Fulham.

You will work with the Hospice @Home Nurse Manager to ensure that all team members participate in professional development and are supervised appropriately within their remit of competency

 Am I the right person for this role?

You will have

  • Experience of working in a Palliative Care either in the community or an inpatient care setting
  • Experience of working with members of a multidisciplinary team
  • Experience of co-ordinating with external teams eg Continuing Health Care, Community Nursing
  • Understand the values our organisation
