Description

To ensure that a co-ordinated, high quality service for patients with lymphoedema is provided and that this is patient focused and will lead to an improved quality of life for the patient. To facilitate the provision of a co-ordinated, high quality service for patients with lymphoedema that is patient focused and will lead to an improved quality of life for the patient. To provide skilled care and expertise in lymphoedema management for all patients referred to the lymphoedema service and to demonstrate competent clinical judgement in deciding patient focused long-range goals or plans.

The Lymphoedema Service operates in two sites. The two lymphoedema clinics are currently based at St John’s Hospice and Charing Cross Hospital and provide lymphoedema support to people living with primary lymphoedema, lymphoedema secondary to cancer, and oedema secondary to any chronic condition or trauma.

Am I the right person for this Lymphoedema Practitioner role?

  • Registered Nurse or State registered Physiotherapist or Occupational Therapist
  • Professional registration
  • Educated to first degree level or equivalent
  • Evidence of specialist knowledge and education Palliative care/oncology qualification
  • Significant post registration lymphoedema skills & experience
  • Palliative care/ oncology experience
  • Ability to work within a multidisciplinary team
  • Good organisational and time management skills
  • Good interpersonal skills
  • Comprehensive understanding of palliative care

What are the Responsibilities of this Lymphoedema Practitioner role?

 To work closely with the Lead Lymphoedema Practitioner in the provision of lymphoedema services throughout the service and inpatient unit of the Hospice.

  • To undertake, independently or with supervision if required, a comprehensive assessment of the lymphoedema patients and use sound knowledge of the physical and psychological effects of lymphoedema, reflective practice and evidence based clinical reasoning to identify the patient’s physical, psychological and psychosocial needs including people with diverse or complex presentations and multi-pathologies.
  • To undertake, independently or with supervision if required, a comprehensive assessment of the lymphoedema patients in order to identify, discuss and agree with the patients treatment goals and plans demonstrating an understanding of the patients goals in deciding or reviewing treatment options including people with diverse or complex presentations and multi-pathologies.
  • To undertake, independently or with supervision if required, a comprehensive assessment of the lymphoedema patients in order to establish the patient’s informational needs, and use appropriate materials to ensure optimum understanding of the causes of the condition, its management and anticipated outcomes of treatment including people with diverse or complex presentations and multi-pathologies.
  • To critically evaluate a service user’s response to treatment and adapt management programmes accordingly.
  • To maintain clinical credibility through practice and to utilise the findings of relevant nursing research and evidence based practice to optimise high quality care for the patient with lymphoedema.
  • To develop skills in the use of equipment necessary for the assessment and evaluation of a patient’s lymphoedema
  • To demonstrate or, if required, develop skills in the use of compression bandaging and identifying patients for whom this aspect of treatment is appropriate.
  • To assess the informational needs of the patient with lymphoedema and to use appropriate materials to ensure optimum understanding of the causes of the condition, its management and anticipated outcomes of treatment.
  • To liaise, when a need is identified, with the appropriate primary care, lymphoedema, medical and multi-disciplinary team in order to ensure long term continuity of care and a co-ordinated, high quality, patient focused approach to lymphoedema management.
