Description

We are currently looking for a motivated and compassionate Bereavement Services Coordinator to join the team in St John’s Hospice.

The Hospice consists of 19 bedded Inpatient Unit, Clinical Day Services, a multi-disciplinary Community Team and Lymphoedema Services maintaining highest standards of palliative care.  Services are provided for people with cancer, HIV and other life threatening illnesses and for their significant others.

Patient are admitted to the Hospice Specialist Palliative Care Inpatient Unit for assessment, symptom control,  terminal care, rehabilitation, respite care and medical interventions from a wide catchment area of five Clinical Commissioning Groups of Central London, West London, Brent, Camden, and Islington.

Located in leafy St John’s Wood on the Jubilee line, minutes from Central London, our luxury private Hospital has an international reputation for providing the highest quality of care to patients and is unique in that all profits fund our onsite Hospice, St John’s.
 
What would be my responsibilities be my responsibilities as Bereavement Services Coordinator:
•    To liaise with the MDT in order to assess and agree support for the patient, carers and families
•    To attend various meetings, as required, to ensure good communication within the multi-disciplinary team.
•    To accurately convey to other members of the multi-disciplinary team appropriate information regarding the work undertaken by the Social Work Department
•    To work in partnership with the patient, family or carer in order to agree what their needs are
•    To provide appropriate support using individual, group or family work
•    To provide support and help in talking to children regarding the implications of serious illness and death
•    To provide information, practical support and advice regarding welfare rights and benefits and other sources of funding
•    To help plan for the safe discharge of patients to appropriate community settings, liaising with community agencies where appropriate in order to activate community resources
•    To contribute towards the maintenance, development and evaluation of the Bereavement Service including Child Bereavement
•    To manage a caseload of families with children who require support.   
•    To develop pathways of referral and support for children whose families are not previously known to St. John’s Hospice 
•    To maintain, continuously develop and improve the service in an imaginative way, which takes account of the identified needs of bereaved children and the communities in which they live
•    To participate in the induction and educational programmes of the Hospice
•    To participate in the learning programmes of students on placement in the social work department at the Hospice.
•    To maintain records and keep statistical information as required by the Hospice
•    To keep up to date with developments within social work practice
•    To assess needs of carers separately from clients where this is requested or appropriate.

Am I the right person for this Bereavement Services Coordinator role?
•    Social Work Qualification, e.g. CQSW, Dip SW, CSS, BA/BSc (Hons) in Social Work.
•    Currently registered with HCPC
•    Experience of social work with  individuals and families
•    Significant experience of working with people facing death and bereavement
•    Working within social care services in the statutory, voluntary or private sector
•    Ability to work effectively as a member of a multi-professional team
•    Ability to work with people who are facing life threatening illnesses, their friends, families and carers
•    Demonstrate skills in assessment, therapeutic and group work
•    Knowledge of grief and bereavement theories
•    Working knowledge of relevant legislation, voluntary and statutory services
•    Ability to advocate on behalf of service users
•    Clear understanding of the issues relating to anti-discriminatory and anti-racist practice
•    An ability to communicate effectively with a range of other professionals and agencies
•    An ability to record accurately and produce reports at short notice
•    Effective workload management
•    Experience of developing a service in response to an identified need
•    Up to date knowledge of welfare rights and benefits entitlements
•    Teaching/Training skills
•    Experience of student supervision
